CORE CARE MEMBERSHIP (ages 18-65)
This is a best fit for someone with 0-2 stable chronic medical conditions who wants time with their physician for strong preventative medicine, stable chronic care management if needed, and quick access to their own physician when new medical issues arise. Core Care Membership does not include ongoing care for hormone packages or mental health care. Members in this plan usually need ~4 or fewer visits per year with Dr. Holt.
This rate reflects the standard cost of a general membership:
$100 non-refundable booking fee, applied toward the $200 cost of the 60-min initial consultation (patients may forego the initial consultation and just pay $200 onboarding fee to get established)

Discounts*:
Family Discount: 2 Adults living in same household enrolling at same time: $220/month
(Families must enroll at same time and pay all monthly membership fees and any additional fees from the same account)

60 minute initial consultation

30 minute follow-ups

One annual preventative care visit per year for cardio-metabolic health

A second annual preventive visit per year for sexual and reproductive health (pap smear, breast exam, testicular exam, etc)

Acute urgent visits with a doctor who already knows you, typically scheduled within 48-72 business hours as needed

Prescription management and lab review

Stable chronic disease management

Sexual and reproductive care (and optional add-on for Peri+Meno package)
WHAT'S NOT INCLUDED:

The cost of labs, procedures, prescriptions dispensed, and imaging ordered by Dr. Holt (can use insurance or pay cash prices for this)

Once it's your turn, we will reach out with a medical history and symptom questionnaire to complete and we'll schedule you for a free 15-minute meet and greet. There, Dr. Holt will recommend the best fit membership plan for you, and schedule you for your initial consultation.
Membership and medical care starts at your initial consultation. We have a minimum enrollment period of 3 months. After that, if you don't want membership anymore, you can cancel without penalty.
