ADVANCED COORDINATED CARE ADD-ON TO MEMBERSHIP
The traditional medical system is increasingly difficult for patients to navigate. But for folks who need more frequent visits or intensive deep dives into complex medical conditions that are new or evolving, it's critical to have a primary care doctor who is committed to getting to the bottom of symptoms and being a bridge to personalized specialty care when needed, because the traditional American healthcare system wasn't designed to do it for you.
The Advanced Coordinated Care Add-On is for members who need medical monitoring and treatment of one complex medical condition or 2 or more medical conditions that might require the patient to have at least a monthly visit at Poppy, or such that Dr. Holt needs to spend extra time coordinating care with specialists or other services (physical therapy, occupational therapy, nutritionist, therapist, etc). The add-on price for this level of care is intentionally higher to protect the time needed to accomplish this level of care.
Some examples of complex medical conditions that tend to require more intensive outpatient time commitment and coordination of specialty care include:
-Hypermobility syndromes (such as Ehlers Danlos Syndrome) and comorbidities
-Long Covid
-Chronic Fatigue Syndrome/Myalgic encephalomyelitis
-Fibromyalgia
-Others not listed
Our philosophy is to be a model for sustainability in the medical care we provide and the business model we've created. We charge what we believe is a fair price so we can give each and every patient the amount of time with the doctor that they need. Rather than charge patients per visit, which can discourage people from seeking care when they need it, our membership model is designed to charge a fair amount for primary care services while also reserving time and space for patients to see the doctor when unexpected health issues arise. We protect time for all our members with same day and next day urgent care availability, we give all our members Dr. Holt's cell number to text if needed, we operate only 5 days a week so we don't overdo it, and we take holidays off. To that end, when a patient has one or more medical conditions that require a necessarily more intensive time commitment from the clinic, we charge a fair correlating fee to protect that time for the patient. Remember, your membership fee is all you pay to see the doctor. We do not bill your insurance company for visits.
With the Advanced Coordinated Care Add-On, Poppy Direct Care offers patients protected time to have more visits with the doctor than they would at a traditional primary care office, it protects the doctor's time to do things like have phone calls with other members of the patient's treatment team or coordinate care in other ways, and it enables us to keep our membership roster small, which protects time for all our patients in a fair and sustainable way.
