If you're ready to sign up, request an initial consultation below. If you still have questions, request a complimentary meet & greet and ask away!

Click the link below to request a date and time for your initial consultation to start the process to join the membership. There is a non-refundable $100 fee for the iInitial Consultation. If you’re a former patient of Dr. Holt’s, or need scholarship-based membership options, please let us know so we can talk discounts. After you request and pay for a visit, we'll confirm the appointment via call or text.

STEP 2
Second Option:

We understand. If you’re new to the concept of Direct Primary Care and still need to talk it through, we’ve got you! Our patients LOVE our practice model. We know many people have questions about whether it's the right fit for a busy person who is used to insurance-based care. We would love to meet (virtually or in-person!) to learn more about you and help you decide if our innovative young adult primary care model is the right fit. Click below to schedule a complimentary meet and greet with Dr. Holt. 

STEP 3
Initial Consultation:
Medical History

Once we confirm your initial consultation appointment, we’ll send you some forms to fill out prior to the appointment. At the consultation, we'll review your intake forms, your medications (and whether we can save you money on your medications by getting them at wholesale prices), give you a tour of the clinic and discuss how it all works. This can be done via telemedicine, but we encourage you to visit our beautiful space. At the time of your intake appointment, if the shoe fits, we’ll discuss whether an Eating Concerns Add-On to your Primary Care Membership makes sense for you. If we both agree it is, we'll schedule your Eating Disorder Medical Stability Evaluation. 

STEP 4
Eating Disorder Medical Stability Evaluation

A medical stability evaluation might take a few days to a few weeks to complete. It includes a comprehensive medical exam, an in-house EKG, urine analysis, vital signs including a weight and orthostatic vitals signs (blood pressure and heart rate in lying, sitting and standing position), and labs. At the end of your assessment, we will give our recommendation for medical care and discuss a treatment plan. Often, this plan includes creating a treatment team consisting of an Eating Disorder specialized Dietician, therapist, and a Psychiatrist if you don’t already have these. We encourage questions and want our patients to understand our recommendations. If your treatment plan includes outpatient care at Poppy for your eating disorder, we'll start the Eating Concerns Add-On to the Primary Care Membership right away and get to work trying to help you get into recovery as soon as possible. The necessity of the Eating Concerns Add-On will be reassessed every 3 months, and the goal is to get rid of the add-on as soon as possible.
