EATING CONCERNS CARE ADD-ON
How do I get in on this awesomeness?

Our practice model is innovative and unique. Peruse our website to learn how things work so you can better decide whether it's the right fit for you. We highly recommend that you read our FAQ section before scheduling your first visit for membership. If you've been diagnosed with an eating disorder or you know you're experiencing disordered eating and you're ready to get help now, you can self-enroll in membership below. During our first visit together, we will either start your eating concerns add-on right away, or we will start with a medical stability evaluation to determine whether outpatient care is right for you at the moment. If you still have questions about how it works, request a complimentary meet & greet and ask away!

First Option:
"I'm game!
Sign me up!"
Click the link below to schedule a date and time for your first appointment as a member. There is a $100 non-refundable booking fee for your first appointment because we fiercely protect clinic time for our current members. Much like buying a concert ticket, we only offer so many new patient appointments per week, and our small business cannot recover from folks not showing up for scheduled new patient appointments. Once you've selected your first appointment date and time, you will receive an email asking you to sign consent forms, including the membership agreement. The first month of membership will bill the night before your first visit, so that once you get to Poppy, you're already in(!) and we can focus all our attention on you and your health goals during your first visit. We will start the eating concerns add-on or medical stability evaluation together during your first visit.
We have a minimum enrollment period of 3 months for primary care. After that, if you don't want membership anymore, you can cancel without penalty.
To recap: you will pay $200 up front before your first visit: $100 non-refundable booking fee for the visit, and $100 to cover the first month of membership, which starts the night before your first visit.

Second Option:
"I still have
questions."
We understand. If you’re new to the concept of Direct Primary Care and still need to talk it through, we’ve got you! Our patients LOVE our practice model. We know many people have questions about whether it's the right fit for a busy person who is used to insurance-based care. We would love to meet (virtually or in-person!) to learn more about you and help you decide if our innovative primary care model is the right fit. Click below to schedule a complimentary meet and greet with Dr. Holt.

Your first visit as a member
During your initial membership visit, we'll review your intake forms, discuss your medical history and your medications (and whether we can save you money on your medications by getting them at wholesale prices), give you a tour of the clinic and discuss how it all works. This can be done via telemedicine, but we encourage you to visit our beautiful space. You are considered a member by the time of your first appointment, so medical care starts immediately. During your first visit, we will decide whether to proceed with the eating concerns add-on or a medical stability evaluation first.

Eating Disorder Medical Stability Evaluation
A medical stability evaluation ($495) might take a few days to a few weeks to complete. It includes a comprehensive medical exam, an in-house EKG, urine analysis, vital signs including a weight and orthostatic vitals signs (blood pressure and heart rate in lying, sitting and standing position), and labs. At the end of your assessment, we will give our recommendation for medical care and discuss a treatment plan. Often, this plan includes creating a treatment team consisting of an Eating Disorder specialized Dietician, therapist, and a Psychiatrist if you don’t already have these. We encourage questions and want our patients to understand our recommendations. If your treatment plan includes outpatient care at Poppy for your eating disorder, we'll start the Eating Concerns Add-On to the Primary Care Membership right away and get to work trying to help you get into recovery as soon as possible. The necessity of the Eating Concerns Add-On will be reassessed every 3 months, and the goal is to get rid of the add-on as soon as possible.
