​This is a concierge-like level of care. Best for patients who require or prefer enhanced access, ongoing support more frequently than a traditional primary care doctor would provide, and/or complex-care coordination. Members in this plan usually need ~8 -10 or more visits per year with Dr. Holt.

Family doctors are trained to take care of many health needs like hormones, metabolic health, sexual and reproductive health, and mental health, but in the traditional insurance-based healthcare system the Primary Care doctor cannot spend the time needed to do this, and patients find themselves referred out to other specialties for basic primary care, like:​

• OB/GYN for hormone prescriptions for perimenopause/menopause

• OB/GYN office for routine well care like a breast exam or pap smear

• Mental health clinic for generalized anxiety, depression or ADHD

• Obesity medicine clinic for help with metabolic or weight management

 

Having one knowledgable doctor who can "keep house" and find the best treatment with all the patient's conditions and concerns in mind requires the finesse of a physician who is capable of treating it all. The DPC model allows Dr. Holt to do just that in the Advanced Care Membership

• With 1+ chronic medical conditions that require frequent check-ins,

• Who wants time with their physician for strong preventative medicine, 

• Who wants a physician who can provide wraparound care for many different concerns because she herself is managing them and/or coordinating care with other members of the patient's care team or specialists.​

• Who wants quick access to their own physician when new medical issues arise​​

Advanced Care Membership does include ongoing prescription management for mental health care and hormone care. ​

The price for this membership is intentionally higher to protect the time needed to accomplish this level of care, and includes a Medical Action Plan (MAP).

DETAILS:

• Initial 2-hour consultation to establish a clinical strategy and roadmap for optimized health.

• 30-60 minute follow-ups

• Lifestyle medical review: Sleep, metabolic profile, nutrition, exercise, sexual desire and comfort, mental well-being, relationships, harmful substances/addictions, effects on hair, skin and bone health.

• Comprehensive care review: Dr. Holt personally reviews medical records from other care providers to consider how specialist treatments might impact the patient's other systems, symptoms, and conditions, and whether the specialist's treatment plan addresses all the patient's concerns or if something requires further investigation. 

• Medical Action Plan in this membership is structured with the expectation to optimize chronic care for a longevity plan. Includes recommendations for optimizing current therapy, next steps including enhanced labs, curated referrals as needed, recommended additional investigation and treatment as well as non-pharmacological approaches with the goal of optimizing quality of life for years to come

• One annual preventative care visit per year for cardio-metabolic health

• A second annual preventive visit per year for sexual and reproductive health (pap smear, breast exam, testicular exam, etc)

• Acute urgent visits with a doctor who already knows you, typically scheduled within 48-72 business hours as needed

• Prescription management and lab review

• Active chronic disease management

• Mental health prescription management

• Sexual and reproductive care including hormone care for peri+menopause
