MEMBER APPOINTMENT BOOKING

SICK or PROBLEM VISIT

Book this appointment if you are experiencing new sick symptoms or a problem such as: cough, runny nose, sore throat, painful urination, fever or chills, abnormal vaginal or penis discharge, injury, lump found, etc.

SEXUAL & REPRODUCTIVE HEALTH VISIT

Book this appointment type for any non-urgent sexual or reproductive health need, such as sexually transmitted infection screening (without symptoms), hormone replacement therapy, PrEP refills, birth control refills or desire to change, chronic pelvic or breast/chest concern.

ANNUAL WELLNESS VISIT

Book this appointment type for any well check, annual physical or sports physical. We will talk about your medical, social, psychiatric, sexual history, and your family's medical history. We will review whether you're due for any screenings such as pap smears.

FOLLOW-UP VISIT

Book this appointment to follow-up on a health issue that you're already working on with Dr. Holt & it's time to revisit, or something changed.

INITIAL MENTAL HEALTH APPOINTMENT

Book this appointment if you have a current mental health diagnosis from another doctor, or you're experiencing non-urgent mental health symptoms and you want to discuss new or continuation of treatment with Dr. Holt AND we haven't addressed this together before.

MENTAL HEALTH MEDICATION CHECK

Book this appointment if you're due for a visit to continue receiving refills for your current medication, if you have questions/concerns about current meds or side effects, or about changing doses or medications.

VIRTUAL VISIT

Book this appointment if you would like to address your concern with a virtual visit. Note that depending on your concern you want addressed, Dr. Holt might still need to see you in person after your virtual visit.

INITIAL DIAMONDGLOW FACIAL

REPEAT DIAMONDGLOW FACIAL

VITAMIN B12 INJECTION

($25) Book this appointment if Dr. Holt has recommended vitamin B12 injections.
